Division wins local collaboratives funding |
Former practice liaison officer, Jenn Boomer, is delighted to return to the division to work alongside collaborative program manager Sandi Hill as the program manager for the local collaboratives initiative.
The division recently won funding to run a local collaboratives initiative with a proposal considered among the best submitted and most likely to succeed. This is a great coup for the division and the highly successful collaboratives program already being run in this area.
To facilitate this new local initiative, a second program manager has been funded to assist program manager, Sandi Hill. The division is delighted that Jennifer Boomer, who previously worked for the division as a practice liaison officer, has agreed to return from Ireland to expand the team.
Jenn is thrilled to be involved in this exciting and innovative program and looks forward to meeting the Hippicratics (Wave 1), Waveriders (Wave 2) and the yet to be named Wave 3.
There is increasing interest in the program from other practices in the division and this new funding will enable 10 additional practices to undertake an abridged version of the program focusing on diabetes and coronary heart disease. Workshops will be held locally, which will suit those GPs who found it difficult to commit to travelling to Melbourne.
To date, 23 practices in the division have benefited from involvement in the National Primary Care Collaborative, a general practice program aimed at improving systems management and patient outcomes. These change leaders represented 39% of practices in the area, which is significantly greater than the 20% required to reach the ‘tipping point’ that achieves rapid spread of change.
