    • Introduction
The division and the Northern Rivers Area Health Service (NRAHS) are working to improve the care of patients with chronic health problems in the community through the State funded Priority Health Care Program (PHCP) and the Commonwealth funded More Allied Health Services for rural areas program (MAHS).

The program will focus on the better provision of health services in the areas of cancer, respiratory and cardiovascular illness.

The objectives of the PHCP are to improve health care for chronic and complex diseases, to reduce the number of unplanned hospital admissions, and to improve the quality of life for patients and their carers.

The division has contributed the $150,000 Commonwealth funding it received in 2001 under MAHS. The NRAHS is providing a total of $2.1 million over a three year period to fund the program.

The agreement sees three NRAHS employed clinical coordinators administered through the NRDGP office. This will enhance effective communication and close cooperation with GPs, the wider community and the area health service.

Where the area’s rural residents need additional services, the division’s MAHS program funding will provide these by training, increasing hours of existing services, or providing additional services through the community health centres in those areas.

PHCP coordinators: (l-r) Cecily Barrack, Kerry Wilcox, Sara Hurren

Respiratory coordinator Cecily Barrack is a physiotherapist with extensive clinical experience in the management of chronic respiratory disease. She has worked in all areas of Lismore Base Hospital over the last 20 years.

Cancer coordinator Sara Hurren is a clinical nurse consultant, who specialises in cancer. She has worked in Lismore to raise the profile of cancer services, cancer nursing and the cancer community since 1998.

Cardiac coordinator Kerry Wilcox is a clinical nurse specialist in intensive and coronary care. Since April 2000, she has been NRAHS coordinator for cardiac rehabilitation services, developing both hospital and home based programs in the area.

Working with a network of service providers in implementing the program, the program provides an opportunity to have a positive effect on the burden of chronic disease.
